To schedule an appointment, call the office and ask for Janet. She will ask you for your credit card to be kept on file when she sets up your initial consultation.
Phone consultations with physician:
60 minute consultation
$200.00
30 minute consultation
$100.00
15 minute follow up or Lab review
$50.00
You need to complete the New Patient Questionaire and fax, mail or email it back to the office to be received by us before your telephone appointment.
Cancellations:
If you are not able to keep your scheduled phone consultation, you must notify our office a minimum of 24 hours before your scheduled time or you will be charged for the consultation time.
